RCA- Root Cause Analysis

To solve any problem, it is required to know exact reason due to which the problem is created.
If the exact reason can not be found, the solutions implemented without proper method may lead to wrong way and it will be realized after implementation only.


Expected benefits & deliverables can not be achieved due to Waste of time, man hours, implementation cost.

There are no.of possible causes which may create the problem.
Always Problem due to no. of out of 6M's.

1. Man (i.e. skill, lack of awareness)
2. Machine (error in machine, machine is not capable)
3. Method (process/ methodology is wrong)
4. Material (wrong input material)
5. Measurement (error/wrong measurement technique)
6. Mother nature ( environmental condition affects)


To analyze Root Causes collect the past historical data & Ask:

1.Why the problem is ? What is the effect ?
2.More defects In which time/ shift/person ? (called Stratification) 
3.More defects In which season ? (called Stratification) 
4.More defects At which station ? (called Stratification) 
5.What types of defects creating the problem ? 
6.Which defects are most contributing & need to be concentrated (called Parato analysis) 
7.To which defects give priority ? (called Parato analysis)
Prepare stratification chart / parato chart & conclusion of all above Questions will give the exact behaviour of the problem & where to trigger.

On the base of the behaviour of the problem and priority take most contributing defects/locations where more defects.
Follow the steps as minimum but not limited to :

1.Arrange Brain Storming session and list out all the possible causes which may create the problem.
2.Prepare the Fishbone Diagram for the possible Root causes.
3.Do Why Why Analysis. In this analysis, ask why to each causes possibly upto 5 times each answers of why.
4.Put all the answers of each why in respective branch in Fish bone diagram.


How to analyze Root Causes

Different type of tools are used for analysis of the problem & Find the root causes :

1. Parato chart.
2. Stratification. 
3. Fishbone Diagram (ISHIKAWA Diagram).
4. Scatter Diagram
5. Brain storming.
6. Why why analysis.
7. More other tools as applicable ....
